A Good Faith Estimate is available upon scheduling or upon request.

I am private pay and do not accept insurance. This means that you are responsible for paying the full fee upfront at the time of your appointment.

If you wish, I can provide a superbill that you can submit to your insurance. Although I can provide a superbill, I cannot guarantee reimbursement. Please contact your insurance company directly to find out more on your coverage.

If you need to cancel or reschedule a session, please provide as much advance notice as possible. If you do not show up for your scheduled appointment or have not notified me at least 24 hours in advance, you will be required to pay the full cost of the treatment as booked.
